Job Description
  1. Provides patient-centered nursing care in an outpatient school-based clinic, according to established scope of practice guidelines and standards of care.
  2. Monitor and support overall student health, including tracking individual health maintenance needs, vaccine records, keeping school health records updated, school environment infection prevention.
  3. Provides triage assessment to walk-in patients, their family members, school staff and others for referrals to Primary Care Provider.
  4. Develops health care plans for students with chronic illnesses and disabilities as needed.
  5. Administers medications, under Provider orders, including but not limited to: Immunizations, antibiotics, pain medications, topical agents, insulin, and prescribed medications for chronic conditions.
  6. Assesses and provides student/patient, parent and staff education on specific chronic condition disease processes, treatment, and self-care, integrating health promotion and preventive aspects of care.
  7. Provides navigation support for student/patient referrals or test requests.  
  8. Performs nursing tasks or skills such as removal of sutures and staples, basic wound care, catheterization, audiograms, laboratory procedures (throat/nose culture, drug screen, phlebotomy – butterfly, venous access), spirometry, basic wound care, etc., with demonstrated competency.
  9. Provides nursing expertise and acts as resource to Medical Assistants. May provide work direction to other staff and/or assist with patient care flow and the provision of services. Proactively participating in staff development by seeking to provide “on-the-spot” learning opportunities, creating in-servicing plans, delivering in-servicing plans for support staff.
  10. Assists with telephone triage and other RN pool responsibilities, as needed.
  11. Assist in maintenance/protection of patient privacy and medical record data collection by:
  • Proactively upholding clinic guidelines and standards related to Patient Confidentiality  
  • Review patient data on chart each visit for necessary health maintenance issues and/or follow-up issues from previous visits
  • Rendering complete, accurate and professional documentation of care rendered, procedures performed, tests administered, tasks completed, results received, and/or other patient related information pertinent to care.
  1. Participate in the Clinic mission to provide high quality healthcare by:
  • Actively participating in department In-servicing and Annual Skills Testing
  • Actively seek independent studies to keep aware and current of nursing issues. 
  • Alerting appropriate staff and/or acting on possible safety or confidentiality issues
  • Offering workflow, safety, morale related improvement suggestions
  1. Comply with SSCHC Infection Control Plan including employee testing.
  2. Maintain additional assigned clinical projects (staff development, Coumadin clinic) in a safe, orderly, and documented manner, with minimal supervision requirements. 
  3. Perform other duties as assigned.
QUALIFICATIONS:
 
  1. Registered Nurse with current license and two years nursing experience; current basic life support certification (CPR)
  2. Supervisory experience or demonstrated supervisory aptitude.
  3. Strong computer skills preferred.
  4. Ability to work autonomously.
  5. Ability to relate well to people from diverse ethnic and cultural backgrounds.
  6. Bilingual preferred (Spanish/English or Hmong/English)
